Lane Violation - Free Throw

It has been around 20 years since I first started playing ball. I watch basketball from NBL to NBA and some euro. I have not once seen a lane violation called on a player beyond the 3 point line. Just seems like this ref had a bee in his bonnet!
I don't believe the rule exists in the NBA, and it didn't exist in FIBA until relatively recently. I still have a rulebook somewhere from back when they first introduced alternating possession (back when it was still a jump ball at the start of the second half), which I checked after being called for a violation from outside the three-point line in a game in 2009, and there was no mention of the rules for those outside the three-point line being any different to the rules for those along the lane. So it came in sometime between 05ish and 09.

I am specifically talking about the players not lined up on the key. As per Anonymous post of Rule 43.2.5 it states players must stay behind the free throw line extended and behind the free throw line until the free throw ends. What counts as the free throw ending though?

who are peoples tips for this weekends games, personally think norwood will come up with the upset and forestville to be too good for west.
